Patient Presentation:
Left leg pain for past 9 days.
History:
32 year old female with no significant past medical history presents with left lower extremity pain from an injury sustained 9 days earlier. The patient reports that she while she was running, she felt a pop in her left knee causing her leg to give out laterally and her to fall down. Since that time, the patient has had excruciating pain in her left knee, swelling from her thigh to her calf, difficulty to both fully extend and flex the joint and also symptoms of footdrop.
Physical Exam:
Left Knee Exam
Mildly swollen
Tender to palpation over both medial and lateral joint lines
Decreased ROM both upon flexion and extension
Extreme laxity to varus stress
No laxity to valgus stress
Positive Lachman
Positive posterior drawer test
Positive dial test at 30 and 90 degrees
Unable to dorsiflex and evert the foot or extend the great toe.
