Patient Presentation:
A 46 year old Hispanic female janitor presented with 2 weeks of bilateral elbow pain.

History:
She reported no injury. It was worse with whipping towels and repetitive motions. She had trouble sleeping because of the pain. She also reported pain in both shoulders and her upper back. The pain radiated into the wrists and hands. She had previously been treated with chiropractic treatment, ibuprofen and acetaminophen, with no improvement. She had landmark-guided bilateral extensor common tendon corticosteroid injections with her primary care provider, with no relief. She was then seen in the sports medicine clinic and was given tizanidine, diclofenac and trazodone. This treatment also gave her no relief. After this treatment she reported continued bilateral pain, waking her from sleep, with no bilateral tingling in her hands.
