Patient Presentation:
A 22-year-old Division One, African American, male football player presented for his pre-participation exam. On the questionnaire, he responded positive to having chest pain and dizziness during and after exercise.

History:
He reported new onset dyspnea on exertion during summer training while he was sprinting alongside his teammates. He had a feeling of discomfort and shortness of breath. He endorsed seeing spots in his eyes requiring him to sit down for ten minutes until he had the strength to walk off the field. He was unable to continue practicing for a few days due to generalized fatigue. He denied any chest pain, headaches, or recent illness.
Past Medical History: Protonix 40mg daily for gastric reflux disease
Family History: Athlete denied any cardiac issues as he was unaware of his family history.

Physical Exam:
General: Alert and oriented, no acute distress
Lungs: Clear to auscultation bilaterally, non-labored respiration
Heart: Normal rate, regular rhythm, no murmur or gallop
Abdomen: Soft, non-tender, non-distended
Musculoskeletal: Full range of motion, 5/5 strength in all extremities
Extremities: No edema, +2 peripheral pulses
Psychiatric: Cooperative, appropriate mood and affect
