Patient Presentation:
KC is a 64 year-old former Marine and Olympic track and field coach who continues to actively coach and participate in master-level bodybuilding and track and field competitions. However, over the last two years, he has been unable to participate in athletic competitions secondary to progressive right knee pain.

History:
The pain was localized to the anterior knee. His pain occurred with impact activities only, including running and jumping, which are crucial for his contests. Walking, cycling and swimming did not reproduce his symptoms. He also complained of intermittent locking and buckling of the knee.

Physical Exam:
The patient had lean, muscular legs. There was no effusion of the right knee. No tenderness to palpation. The patient had a positive patellar grind test but a negative patellar apprehension test.
