Patient Presentation:
Right hip and right low back pain
History:
51-year-old female Pilates instructor with hypothyroidism presented for recent onset right hip and right low back intermittent pain for months without acute injury. Pain radiates from her lateral hip to her posterior hip/SI region and down the back of her leg. Pain worsened with prolonged sitting, prolonged standing, side sit-ups, and direct pressure applied to lateral aspect of hip. Pain limited her Pilates instructing. No bowel or bladder dysfunction. Several years ago she had similar pain that improved with physical therapy.
Physical Exam:
Focused MSK: Tender to palpation at the right sacroiliac (SI) joint, right piriformis, and right tensor fasciae latae. No tenderness to palpation in the groin, anterior hip, or greater trochanteric bursa. Hip range of motion was intact, 5/5 strength in all planes, non-antalgic gait. Negative FADIR, positive FABER with SI joint pain elicited. Sensation grossly intact in bilateral lower extremities. Pedal pulses 2+ bilaterally.
