Patient Presentation:
This 46 year old male presented to the office with severe left shoulder pain. He had pain with any movement of the shoulder and complained of swelling on the front of the shoulder.

History:
The patient states that the shoulder had been sore for a couple of days, but became acutely worse on the day of the exam. There was no history of trauma or overuse. The patient is not involved in any sports activities. He denied any fever or other joint pain. The patient had tried ibuprofen, but was not finding that helpful.

Physical Exam:
Slight swelling over the anterior aspect of the left shoulder was noted. No erythema was appreciated. There was pain with any movement of the shoulder. Tenderness to palpation was noted diffusely over the anterior aspect of the shoulder. Neck exam was unremarkable. Neurologic exam of the upper extremities was grossly normal although strength testing was difficult to interpret due to the patients pain
