Working Diagnosis:
Left (dominant) supraspinatus tear
Treatment:
After confirmation of a supraspinatus tear, the patient received 3 mL of leukocyte rich platelet rich plasma (LR-PRP) injections into the intra-tendinous area and 1 mL of LR-PRP into the subacromial region to help optimize full recovery of the injury Case Photo #2 .
Outcome:
After 6 weeks of rest and physical therapy post injection, the patient was able to return to pitching without pain, instability, or limited range of motion.

Ehlers Danlos Syndrome (EDS) is a connective tissue disorder, often presenting as musculoskeletal laxity and pain. These patients are especially prone to joint subluxations and muscle tears. Specifically, non-operative treatment for rotator cuff tears include as needed over the counter pain medications, physical therapy (i.e. range of motion, stretching, and strengthening of the rotator cuff muscles, scapular stabilization, posture correction program), and orthobiologics like platelet-rich plasma injections (PRP). While PRP has seen utility in treating other musculoskeletal conditions, there are limited studies evaluating its role in patients with EDS.

Beighton hypermobility criteria/score can be used as an adjunct to physical exam to assess hypermobility. The Beighton Scoring System measures joint hypermobility on a 9-point scale. The joints assessed are:
1. Knuckle of both little/fifth/pinky fingers
2. Base of both thumbs
3. Elbows
4. Knees
5. Spine
There are no consensus guidelines in regards to athletic participation in individuals with EDS. Extremity involvement is common with multidirectional instability leading to sublux or dislocation being a concern. Those individuals with vascular type EDS should avoid collision sports. Aerobic exercise and lower impact activities are recommended but decision on participation should be made on a case to case basis.
