Patient Presentation:
A 13 year old female presents with a 3 week history of right knee pain, swelling and popping. She feels like her knee is going to give out.

History:
One year prior she had presented to her physician's office with right knee swelling. At that time an MRI was performed and showed osteochondritis desiccans to the medial femoral condyle with a possible undisplaced loose bony fragment. With this finding the patient was sent to see an orthopedic surgeon who decided to do arthroscopic surgery to her right knee placing a bioabsorbable pin into the loose body and through the medial femoral condyle. She was placed non-weight bearing for 6 weeks followed by a physical therapy program, which she successfully completed. She resumed playing basketball at that time and was pain free until this visit.

PMH: asthma
Social and family histories are unremarkable
Medications: none
Allergies: Nickel, codeine, keflex

Physical Exam:
Weight 130 pounds, height 5'9"; well-developed, well-nourished female in no acute distress. Full bilateral lower extremity range of motion and 5 out of 5 strength. No tenderness to palpation over the medial or lateral joint lines. No patellar apprehension or patella facet tenderness. Negative Lachman's. Negative varus and valgus stress test. Negative McMurray's. There is a 1+ knee effusion on the right.
