Working Diagnosis:
Hypothyroidism

Treatment:
1. Referred for psychological evaluation, which was normal.
2. Referred to Endocrinology after labs obtained for evaluation and treatment recommendations.
3. Diagnosed with Hashimoto’s thyroiditis with subclinical hypothyroidism.
4. Levothyroxine 75 mcg daily was initiated.

Outcome:
Close follow up scheduled with Endocrinology at this time.

Author's Comments:
Returned to play immediately with improvement of symptoms over several weeks and is back to baseline currently.

Unfortunately, athlete transferred to a different school during middle of academic year and follow up has not been obtained.

Editor's Comments:
I'd like to see a reference for Vitamin D and it relation to psychological effects. Based on recollection, it may affect cognitive function in the elderly but I was suprised to see Vitamin D deficiency on the differential and yet substance abuse was not.
