On Segond Thought Not So Pathognomonic - Page #3

Lab Studies:
Right knee x-ray showed a cortical avulsion fracture of the lateral tibial plateau, mild trochlear dysplasia, open growth plates, and a moderate suprapatellar effusion. Case Photo #1 : Segond fracture. Case Photo #2 : Trochlear dysplasia.

Right knee magnetic resonance imaging demonstrated an osteochondral injury to the anterolateral distal femur and a displaced chondral fragment into the lateral recess of the suprapatellar knee joint. A large suprapatellar knee joint effusion was noted. The medial retinaculum, cruciate ligaments (ACL and PCL), collateral ligaments, and menisci were intact. Case Photo #3 : Anterior cruciate ligament intact with large effusion. Sagittal T1 view. Case Photo #4 : Displaced chondral fragment into the lateral recess. Axial T2 view. Case Photo #5 : Possible patellar donor site. Sagittal T1 view.

Other Studies:
No other studies were obtained.

Consultations:
The patient was referred to orthopedic surgery where a more focused history revealed that her current presentation was not a primary dislocation event.
