Patient Presentation:
The patient presented with medial lower leg pain and intermittent numbness from her knee down approximately 2-3 minutes into exercise. Rest immediately relieved the symptoms.
History:
A 20-year-old collegiate lacrosse player had a history of bilateral shin pain that started one year ago. She was diagnosed with bilateral tibial stress fractures. Case Photo #5 She eventually returned to play after taking time off, however, she developed medial lower leg pain, swelling, and tightness with exercise. At times she also had difficulty dorsiflexing her feet bilaterally. Compartment testing was performed and compartment syndrome was found to be positive within the lateral and anterior compartments of the lower legs bilaterally. Case Photo #8
She underwent a fasciotomy of the medial and lateral compartments bilaterally.
A year later, the patient reported having continued medial lower leg pain and intermittent numbness from her knee down approximately 2-3 minutes into exercise; rest immediately relieved the symptoms.
Physical Exam:
GEN-NAD
CV-The right and left distal pulses were normal at rest and decreased after popliteal compression and ankle pumps.
