Patient Presentation:
17 year old male football player presenting with neck pain, dysphagia, and a hoarse voice
History:
Five days ago, the patient reports that he had his football pads rammed into his neck. He reported neck pain after the injury but kept playing. He now presents with neck pain, dysphagia, and a hoarse voice.
Physical Exam:
Head: normocephalic, atraumatic.
Eyes: Pupils equally round and reactive to light and accommodation, pupil size 3 millimeters, conjunctiva/corneas normal, extraocular movements intact.
Ears: Canals without blood or cerebrospinal fluid drainage, tympanic membranes clear, external ears without lacerations.
Nose/sinus: nontender, no drainage from nares, septum midline, no crepitus with motion.
Throat/Oropharynx: Lips, mucosa, tongue normal, posterior pharynx clear, no teeth fractures Face: atraumatic, stable mid-face. Voice exhibited mild hoarseness.
Neck: Trachea midline, no posterior midline cervical bony tenderness, crepitus, or step off; mild left neck swelling noted just lateral to the trachea.
Chest: atraumatic non-tender chest wall, respirations clear to auscultation bilaterally.
