Patient Presentation:
A 42-year-old male presented to Sports Medicine clinic for ongoing multifactorial right foot and ankle pain over the prior 2 years.
History:
The patient had previously seen both Podiatry and Orthopedics, with a work-up showing degenerative joint disease of the 1st MTP, sesamoiditis, an intermetatarsal neuroma, and tarsal tunnel syndrome. He had had no success with attempted treatments including taping, orthotics, steroid injections, and immobilization in a walking boot.
Physical Exam:
On initial evaluation, he was found to have pain with forefoot squeeze and tenderness to palpation between the 1st and 2nd MTP joints, as well as pain with passive range of motion of the 1st MTP. The patient also had a positive Tinel's test in the tarsal tunnel. The rest of his exam was otherwise unremarkable.
