Lab Studies:
Lab studies were unremarkable.

Other Studies:
X-ray of the right shoulder was normal Case Photo #1 . An MR arthrogram of the right shoulder showed no rotator cuff tear, labral tear, or cyst Case Photo #2 . The shoulder MR arthrogram did not give an adequate explanation for the continuous pain with pitching. Images were inspected again and only a small portion of the proximal humerus was visible. An MRI of the humerus was then ordered and a latissimus dorsi tendon avulsion from the humeral attachment with 3cm retraction into the axillary fold was present Case Photo #3 and Case Photo #4 . Extensive myotendinous junction edema secondary to the avulsion was observed (photo 3- arrow, photo 4- dotted box).
