Lab Studies:
WBC 12.4 (76% segs, 11% lymphs). BMP WNL. ESR 85, CRP 14.5.
Arthroscopic cell count 24,293 WBCs with 100 percent seg predominance.
Microscopic evaluation Case Photo #1
Culture grew 4+ Hyaline Mold and negative acid-fast bacilli testing. CSF within normal limits. Coccidioides IgM and IgG was positive. B-D Glucan and Histo Ag were negative. Autoimmune workup including ANA, Ds-DNA ab, C3/C4 was within normal limits. HIV, Hepatitis panel, and syphilis screen was nonreactive.

Other Studies:
CT Thorax with abdomen/pelvis without contrast demonstrated Lytic lesions diffusely along the spine, ribs, pelvic girdle with involvement of bilateral Psoas muscles. Gas in the left shoulder consistent with recent incision and drainage. Case Photo #2
