History:
A 20-year-old female competitive skier presented with electric shock sensations which traveled through her spine and into her legs with particular yoga positions. Within a few months, this sensation became more prominent, especially if she began exercising without warming up. It was also aggravated by running, jumping and stretching. She then developed low back pain with an insidious onset of numbness in her left foot. Over the next several months, this progressed to involve her entire left lower extremity with definite clumsiness in the movement of her left leg. This ultimately led to the discontinuation of vigorous physical activities.
Physical Exam:
Her neurologic exam of her left leg showed mild weakness throughout with mildly reduced rapid repetitive movements. She had crossed adductor reflexes with hyperreflexia, decreased proprioception, and decreased pain and temperature sensation. She was minimally spastic and had an upper motor neuron pattern gait with slight circumduction. There was decreased sensation in the left T9-T11 distribution. The remainder of the physical exam was non-contributory.
